For the fastest response, include the job task, current PPE, relevant standard, workplace environment, known failure mode and whether the decision is for a trial, purchase order, distributor quote or safety committee review.
A short note such as "need gloves" leaves too many variables open. A better inquiry might mention stamped metal edges, current ANSI cut level, glove failure mode, required dexterity, wash expectations and the number of workers in the trial. For eyewear, include indoor-outdoor movement, fogging history, splash risk and whether face shields are paired. For helmets, include overhead impact, side impact, electrical class, chin strap and accessory requirements.
The form does not replace a site hazard assessment. It simply gives the technical desk enough context to prepare useful datasheet references, comparison language and questions your EHS or purchasing team can review before a product decision moves forward.
